Tsunade

Granddaughter of the First Hokage and the finest medical ninja alive, Tsunade fled the village after losing her brother and her lover, drowning her grief in gambling, drink and a phobia of blood. Persuaded by Naruto's stubbornness to return and accept the title of Fifth Hokage, she becomes a fierce protector of the Leaf and the teacher who transforms Sakura.

Thanks to a constantly maintained transformation she looks like a woman in her twenties: pale, fine-featured, with blonde hair worn in two loose low ponytails and brown eyes. A violet rhombus mark sits at the centre of her forehead. She dresses in a grey-green sleeveless kimono top closed with a broad obi, dark trousers and open-toed heeled sandals, with a loose green haori bearing the character for gamble across the back. A green crystal pendant hangs at her throat. When her chakra is exhausted the illusion breaks, revealing an aged, wrinkled woman.

Blunt, hot-tempered and enormously proud, Tsunade drinks heavily, gambles badly and reacts to irritation with her fists. She spent decades running from responsibility after losing the people she loved, developing a paralysing phobia of blood and a bitter contempt for anyone who talks about dreams of becoming Hokage. Naruto's refusal to back down cracks that cynicism. Underneath the gruffness she is a supremely disciplined physician who values life above glory, and once she accepts the hat she proves decisive, protective of her shinobi and willing to gamble everything on people she believes in. She shows warmth mainly through scolding, and hides sentiment behind insults and sake cups.

Granddaughter of the First Hokage and grandniece of the Second, Tsunade was raised inside Konoha's founding family and trained by the Third alongside Jiraiya and Orochimaru, earning the Sannin title after a wartime confrontation with Hanzo. Her younger brother Nawaki died on a mission shortly after she gave him their grandfather's necklace for his birthday, and her lover Dan Kato later bled to death in her arms despite her medical skill. The two losses left her unable to look at blood without freezing and convinced her that the necklace curses whoever wears it. She abandoned the village, took Dan's niece Shizune as her apprentice and travelled from town to town, accumulating gambling debts and a reputation as a legendary loser.

1. Search for Tsunade

   Named as the Third's successor, she is hunted down by Jiraiya and Naruto in a gambling town. Orochimaru finds her first, offering to resurrect Nawaki and Dan if she heals his ruined arms. Torn between temptation and loyalty, she stalls, drugs Jiraiya, and is confronted by Naruto over what being Hokage means.

2. Naruto's wager

   She bets Naruto that he cannot master the Rasengan within a week, staking her grandfather's necklace against his claim on the Hokage title. Watching him collapse and keep going, and hearing him repeat words once spoken by her brother and Dan, she finally places the necklace around his neck herself.

3. Sannin battle and inauguration

   Overcoming her terror of blood, she fights Orochimaru and Kabuto alongside Jiraiya and Naruto, summoning Katsuyu and using a self-healing technique that costs her years of life. She then returns to Konoha, heals Rock Lee, Sasuke and Kakashi, and is formally installed as Fifth Hokage.

4. Sasuke Recovery Mission

   As Hokage she authorises Shikamaru's hastily assembled genin squad to pursue Sasuke, then sends for Suna reinforcements when the mission goes badly. Afterward she performs the risky surgery that repairs Rock Lee's arm and leg, restoring his career as a shinobi.

5. Post-retrieval period

   She governs a village still rebuilding, dispatching teams on missions and managing the growing threat of Orochimaru's Sound village and the organisation pursuing Naruto. She reluctantly approves Jiraiya's plan to take Naruto away from Konoha for an extended training journey.

Naruto Uzumaki

Rival turned protege

She initially dismisses him as a loudmouth chasing an empty title, then recognises in him the same stubborn hope that her brother and Dan carried. She entrusts him with the First Hokage's necklace and afterwards treats him almost as family.

Shizune

Apprentice and assistant

The niece of Tsunade's late lover, Shizune has followed her for years, dragging her away from creditors and eventually serving as her aide once she becomes Hokage. Their bond is part mentorship, part mutual guardianship.

Orochimaru

Former teammate and enemy

He exploits her grief by offering to revive her brother and her lover in exchange for healing his arms. Her refusal, and her decision to fight him instead, marks the point where she reclaims her identity as a Konoha shinobi.

Jiraiya

Former teammate

They trade insults constantly and he never stops pursuing her, but he is the one who convinces her to return to the village. She trusts his judgement in a crisis even while refusing to admit it aloud.
